Below are the new patient forms that must be completed prior to your first session. You can submit these forms from the website; or download them, print them out and complete them by hand. Please then, bring them to your first session. All new patients must also bring with them to their first session, their insurance card as well as a form of ID as well.*

I accept most insurance companies, but please check with your insurance company for exact coverage details (e.g., eligibility, deductible, co-pay, etc.).
